All safety planning activities, risk assessments, interventions, referrals, and follow-up actions should be documented according to C3P protocols and database procedures. Staff should regularly reassess risk, as online victimization can escalate rapidly or re-emerge over time. Safety plans should remain flexible, collaborative, and responsive to evolving survivor needs.
